Community Health Workers (CHW) help individuals and their families navigate and access community services, other resources, and adopt healthy behaviors.
The CHW will help clients identify and address health goals. The CHW provides social support and advocates for clients’ health needs, and provides services such as needs assessments, health education, outreach, support and coordination to obtain needed resources. Works under indirect supervision in community based settings, including clients’ homes. Training will be provided upon employment MDHA offers competitive salaries and an amazing retirement program.
