Leveraging Community Health Workers within California’s State Innovation Model: Background, Options and Considerations

Characterizes the continuum of professions that may provide services/education aligned with the CHW model, including Promotores, patient navigators, and other titles; describes possible approaches to incorporate these professions into the primary care system or into community-based practice models, including reviewing examples from other states; describes possible financing mechanisms to support these professions; and, identifies any evidence of beneficial outcomes associated with this the use of CHWs and associated professions.
