Community Health Worker Model for Care Coordination

Takes an extensive look at the CHW care coordination model, how this model is applied in several frontier states and emerging issues relating to workforce development, regulation, funding and policy. Examples on how CHW and care coordination models are emerging in six frontier states (Alaska, Montana, Minnesota, New Mexico, Oregon, and Texas) are discussed.
