This Notice of Funding Opportunity is noncompetitive and will support state investments in implementing and evaluating evidence-based strategies to prevent and manage cardiovascular disease (CVD) and diabetes in high-burden populations/communities within each state and the District of Columbia, contributing to improved health outcomes. High burden populations are those affected disproportionately by high blood pressure, high blood cholesterol, diabetes, or pre-diabetes due to socioeconomic or other characteristics, including inadequate access to care, poor quality of care, or low income.
