This survey’s intent is to better understand the existing CHW employment environment, providing crucial data data to assist state and local officials, health and human service system representatives, and health and human service payer organizations in understanding the growing array of CHW activities statewide to shape our CHW policy and financing agenda in Michigan.

The survey was designed to answer the following questions: What type of organizations employ CHWs in Michigan and where are they located?; What functions do CHWs serve in Michigan?; Who are CHWs in Michigan, in terms of education, training, and demographics?; How are CHWs trained in Michigan?; How do organizations support CHW employment?; To what extent and how are CHW positions sustained in Michigan?; How do CHWs contribute to evidence-based lifestyle change programs?; How has the CHW field changed in Michigan since 2018?
