Webinar: CHW Toolkit Discussion With Andrea Krotzer

At the heart of CHW work is health coaching by helping others gain the knowledge, skills, tools, and confidence to become active participants in their care so that they can reach their self-identified goals. Join us as we explore the tools in a CHW toolkit including the Motivational Interviewing and creating a health action plan which will explore the steps taken or activities performed that lead to good health while being inclusive of health measures tools that are used to measure patient/client status and compare to the ideal quality of life. The purpose of the tool kit is to keep CHW work client-centered.
