Nomination Form for 2021 Board of Directors Election

 

APPLICATION INSTRUCTIONS

Thank you for your interest in serving on the Board of Directors for the National Association of
Community Health Workers (NACHW). NACHW will hold its election for the Board of Directors
at the 2021 Unity Conference & Annual meeting. All NACHW members in good standing are eligible for nomination. Members may nominate themselves or other members.

In accordance with the NACHW bylaws, the Nominating Committee will recommend a slate of
candidates for the members to elect. The election will be held on July 28, 2021 during the Unity
Conference & Annual Meeting. All members will be voting electronically, as this year’s event
will be virtual. The application and supporting documents are important parts of our
process for developing the slate of candidates. Therefore, it is imperative that a complete
packet be submitted. Incomplete packets will not be considered. Applications are due no later
than May 14, 2021 @ 5:00 Eastern Time.

Nominee packets must contain:

• Completed nomination form – click here to access the form
• Resume
• Candidate statement. If you are nominating a fellow member, please obtain a statement
for inclusion.
• 2 letters of recommendation. One letter must come from a local, state, regional or
national CHW alliance, association, coalition, network, where one exists. At least one (1)
letter should speak to the nominee’s engagement in the CHW movement as an ally or
CHW. All letters should contain contact information of the author.

Nomination forms must be submitted as a packet electronically. Email a copy of the completed
forms and supporting documents to info@nachw.org, using the subject line “2021 Nomination
Form.” Materials received electronically or postmarked after May 14th will not be accepted.
